Sex during pregnancy: What's OK, what's not

Has pregnancy spiked your interest in sex? Or is sex the last thing on your mind? Either way, here's what you need to know about sex during pregnancy.


If you want to get pregnant, you have sex. No surprises there. But what about sex while you are pregnant? The answers are not  always as clear. Here is what you need to know about sex during pregnancy.

Is it OK to have sex during pregnancy?

As long as your pregnancy is proceeding normally, you can have sex as often as you like — but you may not always want to. At first, hormonal fluctuations, fatigue and nausea may sap your sexual desire. During the second trimester, increased blood flow to your sexual organs and breasts may rekindle your desire for sex. But by the third trimester, weight gain, back pain and other symptoms may once again dampen your enthusiasm for sex.

Can sex during pregnancy cause a miscarriage?

Many couples worry that sex during pregnancy will cause a miscarriage, especially in the first trimester. But sex is not a concern. Early miscarriages are usually related to chromosomal abnormalities or other problems in the developing baby — not to anything you do or do not do.

Does sex during pregnancy harm the baby?

Your developing baby is protected by the amniotic fluid in your uterus, as well as the mucous plug that blocks the cervix throughout most of your pregnancy. Sexual activity will not affect your baby. Contrary to Chinese doctors advice stating sex can hurt the fetus. This is absolute rubbish. Sex can not and will not hurt the fetus.

What are the best sexual positions during pregnancy?

As long as you're comfortable, most sexual positions are OK during pregnancy. As your pregnancy progresses, experiment to find what works best. Rather than lying on your back, you might want to lie next to your partner sideways or position yourself on top of your partner or in front of your partner. Let your creativity take over, as long as you keep mutual pleasure and comfort in mind.

In most normal pregnancies at hCG levels below 1,200 mIU/ml the hCG usually doubles every 48-72 hours and it normally increases by at least 60% every two days.

In many early pregnancies, the hCG level will double every 31 to 72
hours. As the pregnancy progresses, hCG levels rise and the doubling
time increases.

In general, after 6-7 weeks of the pregnancy the best indication of a healthy pregnancy is a good fetal heartbeatBlood hCG levels are not very helpful to test for the viability of the pregnancy if the hCG level is well over 6,000 and/or after 6-7 weeks of the pregnancy. Instead, to test the health of the pregnancy bette at that timer, a sonogram should be done to confirm the presence of a fetal heart beat. Once a fetal heart beat is seen, it is not recommended to check the pregnancy viability with hCG levels. 


Under 1,200 mIU/ml Between 30 and 72 Hours
1200-6000 mIU/ml Between 72 and 96 Hours
Over 6,000 mIU/ml Over 96 Hours

hCG levels rise during the first 6 to 10 weeks of pregnancy then decline slowly during the second and third trimesters.

As your pregnancy develops, the increase slows down significantly. Between 1,200 and 6,000 mIU/ml serum, the hCG usually takes about 72-96 hours to double, and above 6,000 mIU/ml, the hCG often takes over four or more days to double. It makes little sense to follow the hCG values above 6,000 mIU/ml as at this point the increase is normally slower and not related to how well the pregnancy is doing.

After two to three months the increase will slow even further and eventually hCG levels even decline before reaching a plateau for the duration of the pregnancy. Doctors will often use the quantitative blood test if they are closely monitoring the development of a pregnancy but above a level of about 6,000 mIU/ml that makes no sense because at this point a sonogram provides better information about the pregnancy.

One single hCG value does not tell you much about the viability of the pregnancy and there is a wide range of normal hCG levels in pregnancy. A normal hCG rise over several days prior to 6 weeks of the pregnancy usually indicates a viable pregnancy. In general, the best indication of a healthy pregnancy other than a normal rise of hCG is a good fetal heartbeat after 6-7 weeks.

Bleeding and spotting during the first 2-3 months of your pregnancy is a
very common event and is seen in about 1 in 3 pregnancies. Though vaginal bleeding during the first 20 weeks without any other signs like passing tissue or cramps is also known as a “threatened abortion”, most women with early spotting will go on and have a healthy baby. “Threatened abortion” means that it is possible that the early bleeding could be an early indication of a miscarriage. An actual miscarriage is more likely if there is bright red blood, usually heavier than a period, together with cramping.

A miscarriage can happen over a period of several days, but the pregnancy in most women with some bleeding and a threatened abortion will continue normally and they will have a healthy baby.

When you are bleeding in the first 2-3 months and especially if you have pain in the lower abdomen, then the first thing to exclude is an ectopic pregnancy, a pregnancy outside the uterus. If you previously had a sonogram and your doctor saw the pregnancy inside the uterus, then an ectopic pregnancy is usually not present.

Many women who bleed are concerned about the possibility of miscarriage and are reassured by a normal test.

Before the 7th week, especially shortly after you miss your period, a normally rising hCG (pregnancy hormone) titer is a reassuring sign that the pregnancy is progressing normally. You can check our hCG tool to verify that your hCG titer is rising normally.

The fetus' heartbeat can usually be seen after the 7th week of pregnancy, so seeing the fetal heart beat on ultrasound after the 7th week is very reassuring but it's no guarantee that it’s not a miscarriage and that you are not in the process of having a miscarriage. Even if you see the fetal heartbeat, a miscarriage may sometimes develop over several days and become evident only a few days later. If it makes you feel more comfortable to see the fetus' heartbeat then let your doctor know and ask to make arrangements for a sonogram.

The cause of a miscarriage in the first trimester is often poorly understood. It usually  indicates a faulty pregnancy, usually an abnormal implantation or an abnormal fetus. In either case there is no treatment.

Many doctors suggest that patients with a threatened miscarriage or bleeding stay in bed. Staying in bed may decrease the bleeding but no study has shown that this in fact prevents a miscarriage. Unfortunately, at the present time there is no definite treatment for this kind of bleeding in the early stages of pregnancy.

Avoid raw, undercooked or contaminated seafood

To avoid harmful bacteria or viruses in seafood:

  • Avoid raw fish and shellfish. It's especially important to avoid oysters and clams.
  • Avoid refrigerated smoked seafood, such as lox. It's OK to eat smoked seafood if it's an ingredient in a casserole or other cooked dish. Canned and shelf-stable versions also are safe.

Avoid undercooked meat, poultry and eggs

During pregnancy, you're at increased risk of bacterial food poisoning. Your reaction might be more severe than if you weren't pregnant. Rarely, food poisoning affects the baby, too.

To prevent foodborne illness:

  • Fully cook all meats and poultry before eating. Use a meat thermometer to make sure.
  • Cook hot dogs and processed deli meats, such as bologna, until they're steaming hot — or avoid them completely. They can be sources of a rare but potentially serious foodborne illness known as listeriosis.
  • Avoid refrigerated pates and meat spreads. Canned and shelf-stable versions, however, are OK.
  • Don't buy raw poultry that's been pre-stuffed. Raw juice that mixes with the stuffing can cause bacterial growth. Frozen poultry that's been pre-stuffed is safe when cooked from its frozen state.
  • Cook eggs until the egg yolks and whites are firm. Raw eggs can be contaminated with the harmful bacteria salmonella. Avoid foods made with raw or partially cooked eggs, such as eggnog, raw batter, hollandaise sauce and Caesar salad dressing.

Avoid unpasteurized foods

Many low-fat dairy products — such as skim milk, mozzarella cheese and cottage cheese — can be a healthy part of your diet. Anything containing unpasteurized milk, however, is a no-no. These products could lead to foodborne illness.

Unless these soft cheeses are clearly labeled as being pasteurized or made with pasteurized milk, don't eat:

  • Brie
  • Feta
  • Camembert
  • Blue cheese
  • Mexican-style cheeses, such as queso blanco, queso fresco and panela

When selecting eggs, consider buying the pasteurized variety. In addition, avoid drinking unpasteurized juice.

Understanding what foods to avoid during pregnancy can help you make the healthiest choices for you and your baby.

Avoid seafood high in mercury

Seafood can be a great source of protein, and the omega-3 fatty acids in many fish can promote your baby's brain development. However, some fish and shellfish contain potentially dangerous levels of mercury. Too much mercury could damage your baby's developing nervous system.

The bigger and older the fish, the more mercury it's likely to contain. The Food and Drug Administration (FDA) and the Environmental Protection Agency (EPA) encourage pregnant women to avoid:

  • Swordfish
  • Shark
  • King mackerel
  • Tilefish

So what's safe? Some types of seafood contain little mercury. Although concerns have been raised about the level of mercury in any type of canned tuna, the FDA and EPA say pregnant women can safely eat up to 12 ounces (340 grams) a week. Similarly, the 2010 Dietary Guidelines for Americans recommend 8 to 12 ounces of seafood a week for pregnant women. That's about two average meals of:

  • Shrimp
  • Crab
  • Canned light tuna (limit albacore tuna, chunk white tuna and tuna steak to no more than 6 ounces, or 170 grams, a week)
  • Salmon
  • Pollock
  • Catfish
  • Cod
  • Tilapia

Not all researchers agree with these limits, however, citing a study that noted no negative effects for women who ate more seafood than the FDA-approved guidelines.

Your developing baby
Although you reach your estimated due date this week, not all mothers (especially first timers) give birth around that date. First time mothers averagely go over their due dates by eight days and second time mothers by three days. At your antenatal appointment this week you will be checked just to make sure you and your baby are doing well.

Your baby probably measures around 14 to 15 inches in length and is now completely ready to be born. When you meet your baby you may be surprised by how he looks, as newborns don’t often resemble chubby picture-perfect babies, because they are often swollen from their journey down the birth canal. They are more likely to be born with puffy eyes, (sometimes crossed-eyes) rashes and misshapen heads from their squashed journey. Don’t worry, all these conditions are minor and will disappear a few days after birth.

Your developing body
You are probably feeling big, frustrated and anxious to meet your baby. Read up about ways you can kick-start labor at home, from nipple stimulation to walking in the park! You may also be worrying about what contractions feel like and whether you’ll know you are in labor. The general rule is; if you can’t speak during a contraction – you are in labor! When your contractions get to five minutes apart and last for 45 to 60 seconds it’s time to go to the hospital.
The latent stage of labor can last for a good few hours so make sure you have your birthing ball and TENS machine ready and waiting.

Most women go into labor about 24 hours after their waters have broken and sometimes labor can start without the waters having ruptured. If this is the case the midwives will probably break your waters for you. If your waters break at home, phone the hospital to inform them and make a note of what they look like, how much water there is and whether there is a green/ black color to them as could mean your baby has passed a meconium stool and you may need medical attention.

Lots of women go overdue, especially with their first baby. Read up about what happens when you are overdue, and any methods, such as a membrane sweep, are used to help labor along. Keep taking short walks every day as activity can help to encourage your baby into a good position for birth.

You may not feel like it but sex can also help because female orgasms stimulate the first uterine contractions and semen contains the hormone prostaglandin, which softens the cervix, so having sex can be a double labor-starting whammy. On the downside, sex can be pretty difficult when your belly gets in the way, and you may not be feeling particularly sexy. Try lying on your side with your partner behind you, or on all fours with plenty of pillows to support your knees, or a bed or chair in front of you to support your forearms.

This is your last attempt to have a meal out with your partner before your baby (and mum-in-law, cousins, long lost aunties…) arrives. Enjoy each other’s company and give yourselves pats on the back for having already come so far on this amazing journey into parenthood. Oh, and if you do have a meal, make it a curry, because some women swear spicy foods helped to kick-start labor

You are thirty-nine weeks pregnant

Now your feet have disappeared you probably find yourself bumping into things and stubbing your toes. Don’t worry, very soon you will be able to pedicure those nails again… You will probably have trouble performing simple tasks this week without huffing and puffing or having to sit down and rest every five minutes. Your baby’s digestive system has matured enough to be able to take liquid foods and now is a good time to read up about breastfeeding or bottle-feeding to prepare yourself for the coming months. Most of the hair that has covered and protected your baby’s skin has fallen off and she will be born with pinky red skin. She is probably about 7lbs this week and continues to put down fat underneath her skin in preparation for life in the outside world.

Your due date is close now, but Babies go overdue all the time especially for first time mums. Current research shows that first time mothers gave birth on average five days after their due dates and second time mothers gave birth on average two days after their due dates. The reason that inductions happen is that there is an increase in problems for babies that go beyond 42 weeks. This is because there is a very small percentage of babies who die or who are stillborn when the pregnancy exceeds 42 weeks. Doctors don’t know why this happens but feel that it is safer for the mother and the baby to induce labor before 42 weeks. In most pregnancies labor will start somewhere between 37 and 42 weeks and if you go over your due date you will be monitored very closely and will probably need to have weekly antenatal check-ups.

When and why might I need an induction?

An induction is usually recommended when it is felt your baby is better off outside of the womb than in. This may occur if your pregnancy is over 41 weeks, your waters break but labor does not start (your baby is at risk of infection once the waters break), or if you are suffering from a pregnancy related problem such as Pre-Eclampsia where the only cure for mum and baby is immediate delivery. Other reasons include poor growth of your baby or unexplained bleeding at term.

What happens in an induction?

When labor starts naturally your body makes a hormone called oxytocin which is released into the bloodstream and hormonal changes in the body start the process of labor. Nobody knows what the trigger is that starts labor spontaneously (or doesn’t start in some women) although lots of research has been dedicated to the subject. Inductions are when labor has to be started artificially and a synthetic form of oxytocin is administered to bring on labor.
